Summit Area Public Foundation $60,000 Grant Supports SAGE Guidance

SUMMIT, NJ - The Summit Area Public Foundation (SAPF) has awarded a $60,000 grant to SAGE Eldercare.  The grant will support SAGE's Guidance programs.

“We are very grateful to the Summit Area Public Foundation for their generous support of our InfoCare, GPS Services, and education programming," said Angela Sullivan, Interim Executive Director of SAGE Eldercare.  "The grant will allow us to continue to provide innovative services and programs that meet the changing needs of older adults and families in the area who are choosing to age at home."

SAGE Guidance is a comprehensive approach to reaching older adults and caregivers in their homes, and helping them thrive with long-term wellness and independence, through social work support and free referrals, medical care planning, and a full calendar of educational programs that responds to the communities’ changing needs.

SAGE Guidance is a cluster of three focus areas including InfoCare, GPS Services, and public education.  InfoCare’s licensed social workers and registered nurses personally usher the most vulnerable seniors toward long-term wellness, in addition to providing free local and long-distance information and referrals.  
 
Guidance, Planning and Support (GPS) Services works with area medical providers and residents in need of support for older adults experiencing healthcare transitions, and the program helps to increase stability and reduce unnecessary hospitalizations.  
 
The public education events respond to the challenges experienced by clients in each of these programs, with continuous presentations and monthly support group meetings.
 
The SAPF is a non-profit community foundation that fosters philanthropy in the Summit area by identifying local needs and offering donors flexible ways to make a difference in the lives of their neighbors.  
 
The foundation administers three main types of funds: general unrestricted, restricted-purpose, and donor-advised, all of which support not-for-profit agencies in Summit and nearby communities.  The SAPF also awards scholarships to students attending Summit schools.
